I have 4 residual tooth roots in extreme pain, dentist wants to clean them before the extraction, he says it will prevent infection during extraction but i don't know how to handle pain during the cleaning. how can i accomplish that? is it necessary?

You will be locally anesthetized for the extractions. The removal of calculus on those roots before extraction is prudent to prevent pieces from falling into the extraction sockets during the procedure. No worries.
