Second degree AV block (Mobitz type I or II?) can cause pauses in one's heartbeat, requiring a pacemaker. This scenario usually would not cause hypertension. Therefore, one should undergo a careful history, physical exam and lab testing to determine the underlying cause and best treatment for the hypertension.

The pacemaker insertion would not cause by itself a sudden increase in blood pressure. Related issues, such as pain or anxiety may contribute to high blood pressure. Letting his doc know would be best in this context as to getting some clarification and a good plan.
