The most common cause of this sort of thing is hydrocele, a cyst filled with fluid. Hydroceles usually are harmless, but can be removed or otherwise treated if large or otherwise bothersome. There are other possibilities as well, including various infections, tumors, and more. No OTC medicines will help at all and might delay necessary health care. See a urologist soon. Good luck.
