My mom was recently dx'ed with high bp(188/101). dr put her on 50mg losartan. diastolic not moving. dr upped dose to 100mg/day. not budging. endo chimed in and added hydrochlorothiazide. shouldn't they address her diastolic more aggressively?

I assume your mom is above 70 years old. It is safer to lower her blood pressure SLOWLY for both numbers. Dose escalation and adding thiazide diuretic are the appropriate move. If the Hctz dose is already 25 mg, the next step will be adding a calcium channel blocker if her BP is not lower than 150/90 after 4 weeks of Losartan-Hctz (100/25). Secondary hypertension should be ruled out.
