No, if you are asymptomatic. Sinus arrhythmia is commonly seen in healthy people. The normal R axis ranges from -30 degrees to +100 degrees although some sources use the -30 to +90 degrees range. However, if you have other symptoms such as shortness of breath on exertion or in supine position or history of blood clots then echocardiogram should be done for ruling out pulmonary hypertension.
