The term "mildly enlarged" have have been true for both ultrasounds, just a comment by one observer which could have been only marginal to the other. At age "46" there is more estrogen produced secondary to aging eggs, which stimulates pituitary hormones to force ovulation, resulting in what was observed by the ultrasound.
