Muscle spasm is often painful or at least discomfort so you may mistaken spasm with fasciculation or muscle twitching by lay term. At times, the colon contraction may give you the same feeling but you can observe the fasciculation (muscle twitching) but not the bowel contraction at that area. I couldn't think of any condition which causes both chronic hyperbilirubinemia and fasciculation in adults

All laboratory results need to be interpreted in the clinical context and the doctor who ordered the tests is usually in the best position to do that. Talk to the doctor who ordered the tests. Having said that, it would have helped had you provided the actual bilirubin value. Your symptoms and "bilirubin" may be unrelated. Wish you good health!
