Could be physiologic gynecomastia. Have him examined by a pediatrician, who will determine whether he is prematurely developing puberty, precocious puberty.

The pores of a nipple can be invaded by germs like staph and cause a bacterial infection. One sided change as described could be that or it would be 4 years early for hormonal breast change that occur in 1/3 of males. Either would be worrisome & deserve an evaluation by a pediatrician. Infection needs treatment, premature hormonal changes deserve close monitoring.
