Who gets acute renal failure or hepatic injury from angiotensin receptor blockers? if you have normal liver and kidney function before your start an arb when is it most likely to occur and how frequently should you have blood/urine tests to check?

Liver injury from any drug is a fact but it's extremely rare with ARB drug. I have prescribed ARB drugs for more than 20 years (losartan is the first ARB drug) but I never encountered one case. ARB by itself should not cause ARF in a normal, healthy person. If ARB is prescribed for high BP, doing a comprehensive chemistry panel after 3 months is more than enough, no need for urinalysis.

It is a very rare happening which is unpredictable in otherwise normal people. More chances in a person with an already compromised liver or kidney function
