Neck pain 1yr must hyperextend head till loud crack occurs to release pressure/pain in atlanto-occipital region. head heavy. crepitus/snapping sound/feeling w pain in cervical neck. poss craniocervical instability? mri-wnl, 3 diff docs, no ans! help?

In view of an essentially negative workup, I suspect chronic tension in occipital muscles. The cracking sound crepitus is a nondiagnostic feature as far as pathology is concerned. If you think chronic stress or tension is at play, consider stress management therapy and relaxing techniques which I think will help relieve your symptoms.
