A member asked:

Do doctors document issues or complications during birth? looking to obtain anything in writing, of possible complications. this could help in a diagnoses. who would i contact?

The medical records of a newborn would usually list both the normal and any unusual issues encountered in the process. The medical records of the hospital would have that documentation which could be obtained by writing/going to that department, providing written authorization to release the information and paying the cost of creating copying the documents.

Yes, medical issues at birth are always documented, however such information is retained for limited time by medical facilities, around 7 years. Start with medical records at the facility where delivery took place.
