M. hominis : I would first try to determine if its a colonization & if the symptoms are caused by another process or a more common bacteria. For adults with genitourinary tract infections tetrcyclines (eg doxycycline) is preferred. Fluoroquinolones & Clindamycin are alternatives. Treatment duration depends on multiple factors such as site of infection. For eg, bone infection may require weeks of IV antibiotics.
