Is it possible for an untreated keratoacanthoma to still be present after 3 years or is this concerning? it was diagnosed by appearance and growth-shrinkage pattern, but no biopsy was done?

Yes an untreated Keratoacanthoma will remain pleasant for years if not removed by surgery or freezing it with cryotherapy. A biopsy is needed for a definitive diagnosis. See a primary care physician or dermatologist for diagnosis and treatment.
