4.5 years old, ast(sgot) is 61, is that a big issue? he is going to do a tonsillectomy, is it safe to do that surgery with higher sgot? can higher rate of sgot occur because of antibiotic for long period?

I don't consider the value elevated enough to suggest a health problem. These can rise transiently after common viral infections and settle down weeks/months later. It's worth a follow-up test at some point but I would not alter any needed medical procedure based on this value.
