Docs use lubrication for the rectal exam, however hemorrhoids are not manipulated (pushed back) during the exam.

External hemorrhoids belong on the outside and will not stay if pushed inside. Internal hemorrhoids can protrude outside but can gently be reduced internally and should be so. If you are having problems with this figuring out why, ie constipation/diarrhea, then resolving those issues is the best approach.

It will depend on the findings and the judgement of the doc. yes sometimes they do and may use a local anesthetic ointment.
