Halitosis can be caused by many things, including cavities, periodontal problems, acid reflux, sinus infections, and diet. Since you've had this problem for a while, the question is have you seen your physician and dentist to be evaluated? If not, now is the time for consults. Please make the calls.

You have poor oral habits and poor diet causing all of your problems. You need to visit a dentist for x-rays and treatment .Your bad breath and taste is from diet and possible gum disease, your tooth pain from cavities.

Tooth brushing alone is not the answer. When was last dental exam, physical exam, dental cleaning. When was last time you flossed. When were your last lab tests, dental x-rays, etc. Please take responsibility. See both Dentist and a Physician. The causevof your problems is probably neglect.
