I have had a localized area on my upper lip for months that constantly bleeds, crusts/scabs, repeats. i saw an ent and after removal of scabs theres no underlying lesion. however, she recommends still excising the area for biopsy. is this necessary?

Yes. A non-healing ulcer suggests a basal cell carcinoma and it would be prudent to get a proper diagnosis by having the lesion excised and tissue examined by a pathologist. It would be important to involve a plastic surgeon to make sure that excision does not cause a cosmetic issue. Wish you good health!
