Heterogenous parenchyma by ultrasound means an abnormality in hepatic (liver) structure. In most case, this finding should be followed with other tests to determine the cause of the heterogeneity. If you are overweight with history of hyperlipidemia or diabetes mellitus & high liver enzymes, fatty liver is strongly suspected with this finding. You can disregard the notion lymph node and artifact.
