To help him you need for him to see a doctor and medical help for proper care. I hope he can recover. Whether he is dying or not can only be determined after medical exam.

Yes, he is obviously declining. Needs a thorough medical exam, tests as well. While bringing him the bottle might help with alcohol withdrawal, which may be life threatening, it does not sound as a good long term plan, given decline.
