You have mild to moderate anemia and the cause is likely iron deficiency with low MCV, MCH and high RDW. You need to have an iron panel (Iron panel, TSAT, TIBC, Ferritin) done for confirming iron deficiency. If iron deficiency is confirmed, source of blood loss needs to be investigated, abnormal vaginal bleeding and GI bleed are the most common etiologies at your age range.

Agree with Dr. He. You may take oral iron and multivitamins to address your iron deficiency anemia while other issues are being addressed. For healthy diet: http://www.choosemyplate.gov/., exercise 30 minutes/day, drink enough water daily so your urine is mostly colorless, have safe sex, no tobacco alcohol weed or street drugs.
