Yes. Such puncture wounds can drive germs from the nail or shoe deep in the tissue where they can start a deep infection in the fibers/ligaments/bones of the foot. Clean it out well and do not hesitate to go see your doc. A tetanus shot would be reasonable if none in the past 10 yrs. This can get serious fast. If infection starts deep in your foot, it can lead to hospital care/surgery.
