This decision must take into account: what caused the rusty carotids? Is that being treated to stop/reverse the rust? Stop smoking , get BP down to less that 120 systolic. Was the stroke on the one that is completely blocked or the other one. How averse are you to dying on the table. So discuss the risks and benefits with your team and even get a second opinion if not satisfied.

Yes, you are at risk, BUT there is abundant evidence that either medical or surgical approaches might be equally effective in many of such cases. Each is different and no specific individual should be handled similar to another. Would suggest an opinion from a local medical school stroke team, and wish you the best, as this is very complex.
