Fatigue could be a chronic symptom of systemic lupus erythematosus but not usually a flare up (acute) symptom or sign. Steroid has serious side effects so your doctor does not want to use it for your fatigue which is somewhat subjective. However, if serology tests truly confirm active lupus or a flare up, steroid or other immunosuppressive drugs can be used because plaquenil alone is not enough.

A goal of treating lupus in the 21st century is to achieve remission with minimal steroid use. Depending on the organ involved, options include Imuran , Cellcept, Methotrextate ,Benlysta, Saphnelo and Lupkynis
