Type 2 diabetes should be controlled first with diet (less carbs) and lifestyle changes (more exercise) so that the A1c is less than 7.0. However, typically the first drugs prescribed are oral pills such as Metformin and sitagliptin. Insulin injection is often prescribed if the blood sugar and A1c are too high, while continuing diet/lifestyle changes and oral pills.
