I'm a fan of tapering off anticoagulants over several days.to avoid rebound clotting problems. This is most important in patients who have been on them long term. In my opinion his is more of a problem with the novel anticoagulants like xarelto than old fashioned warfarin which wears off more gradually in my experience.
