Can a pulmonary embolism last 3-4 years if left untreated?

No. Pulmonary thrombi can kill instantly if large, multiple small one cause high blood pressure in lungs and heart failure. A thrombus/embolus gets resolved/organized in a matter of day and does not last "years".
