What is the best way to treat mild stroke and numbness in the left side of the body after being discharged from the hospital for about three weeks now?

Imperative to clarify the underlying cause of the stroke, especially in an age 34 yo female. (Is there a history of migraine, associated with smoking, and hormones; is there homocystinuria; are there vascular anomalies??) Anticoagulants are indicated if a vascular dissection has occurred. Antiplatelet drugs may be indicated. Discuss appropriate steps with a neurologist. Causes vary

Physical and occupational therapy are commonly used following strokes in order to compensate for the loss of function, your neurologist will be best to explain and coordinate at followup.
