I'd first talk with your employer, and let them know you're supposed to have a doctor's appointment tomorrow. It's a necessary courtesy, as your employer likely will need to arrange for someone else to fill in. I don't know the details of your work arrangements. If your work OK's it, I'm sure the doc can give a note if necessary. Otherwise, you may have to reschedule. I hope this helps.

Call or e-mail the office or clinic and request a fax or e-mail detailing your appointment and time, with a copy to your HR dept or boss. Once received ask for permission.
