Start with your primary care doc or pediatrician who would be able to refer you to a local weight loss program. Discussions will be escalated to bariatric surgery if needed and meeting criteria.

I would spend the $ and get a 1 month subscription to uptodate.com and read all they have about such surgery. They review the risks and benefits nicely. Then you and your Dr can discuss the risks and benefits and the 2 of you can make an informed decision about what to do.

I send patients with these questions to the surgeon who I recommend for bariatric surgery. In this manner the person gets all the very practical information necessary to make that important informed decision whether to go ahead or not with the procedure.
