First, that's extremely rare; it almost never happens that HIV infected persons do not produce antibodies to the virus. Second, the 4th generation tests indeed would detect infection in such cases. That's their whole point: by detecting both antibody and HIV antigen, after the first few weeks, they are NEVER falsely negative.
