Autoimmune disease, chronic infection should be considered. A diagnostic workup may include an autoimmune panel, inflammatory markers and CBC, differential, platelets.

Be sure to have a specialist in infectious disease involved in your workup. After this assessment you may he referred to a rheumatologist for more evaluation. In my experience cases of fever of unknown origin are best worked up in a hospital where testing and consultations with other specialists such as rheumatologists, hematologists, oncologists will see the patient also without delay.
