Key issue is the effect of the disc rupture, i.e., is there progressive muscle atrophy and weakness, increasing pain, problems with bladder or sexual function? Any of these might suggest surgical consultation. An EMG test would objectify extent of nerve dysfunction. Physical therapy and McKenzie Back Exercises might allow stabilization. I think a neurosurgeon would be best to see at this point. The symptoms may not be all explainable by herniated discs and peripheral neuropathy may be playing a big role here. The neurosurgeon will conduct an examination and review the MRI. An EMG, diabetes check & Vit B12 level should be helpful. Treatment options can then be discussed. Is alcohol excess a problem?
