61-year old male. lump on back of neck, left side, about 1cm below hairline. less than 1cm long, about .5 cm wide, palpable when in regular seated position. when bent forward, pliable, not fixed. noticed 3+ years. no apparent change in size?

Sebaceous cyst versus lipoma, but cyst is more common at that location.

In addition to a cyst and lipoma, it could be an enlarged lymph node. You may feel the lump every couple of weeks, if the lump doubles in size, becomes painful, ulcerates, or other lumps appear, or you develop fever, or lose weight, you should see a doctor. Wish you good health!
