X-ray: sclerotic changes & arthritis in bilat. si joints. i have had stabbing pain in left si joint with jumping/jogging/coughing/sneezing/straining/ downstairs for 1 year. conservative tx & steroid injection didnt diagnose or lessen pain. what next?

I would discuss with your Dr and assume you have given good old regular aspirin a trials. In addition I would avoid jumping/joging/coughing/sneezing/straining/downstars for 2 weeks and see if it gets better. Suspect your body is talking to you. Listen and take good notes and discuss with your team and you all will get to the bottom of it and make it better.

Assuming this is the pain generator, you may be able to handle this symptomatically by wearing a Sacroiliac Belt when up and about. Available online, and inexpensive. Osteopathic manipulative therapy may be very helpful. SI joint stretches may also alleviate some discomfort.
