Nexplanon releases a steady amount of hormone over time which does gradually decline. The hormone levels should still be more than sufficient in the third year though a breakthrough ovulation is slightly more likely to occur in the third year than in the first year. No birth control method is 100% effective, but Nexplanon is very good, even in the third year.
