Description does not fit the typical appearance and behavior of hemorrhoids. She still may have hemorrhoids, but this is not what is causing her current symptoms. An exam will be needed, an infection from the herpes/ shingles spectrum appears likely among others. Starting with her doc/ Obs-Gyn will be best, an urgent care would be next as pain will need to be addressed as well.
