Fibromyalgia comes to mind here. A rheumatologist is best to consult for evaluation. I prefer a consultation with a neurologist for evaluation of headaches,muscle twitching,vertigo. Appropriate testing will be ordered by these specialists.

A few possibilities may explain some of this, such as fibromyalgia and diffuse small fiber neuropathy, but this may not explain all of this. Would start with an internist, and after basic evaluation, might have a neurologist provide additional input. Once diagnosed, appropriate therapy can commence.
