This is quite common, more often in some than others. Increased intra-abdominal pressure from straining at stool, plus stool passing over the prostate gland and seminal vesicles, squeezes out semen and/or prostate fluids. It's doesn't indicate any health problem in absence of other symptoms, such as penile discharge at other times. But can be more often with enlarging prostate with age.

Straining could expel genital secretions making urine appear cloudy. You may take a fiber laxative like Metamucil and drink enough water daily so that your urine is colorless. For the long term you may try to increase your intake of fiber foods.
