It depends: If a pilonidal sinus becomes infected and develops into an abscess, you can develop fever , fatigue etc.

Possibly: Chronic infection starting in a pre-existing sinus or cyst could be minimally sore, cause little drainage and still result in fatigue. But as the only symptom, its not the most likely cause and too many conditions could lead to fatigue. Since you feel it is severe, id see your doctor for a check-up.
