A major part of making a diagnosis is obtaining a thorough history of your illness. This includes time of onset, clear description of the disorder, frequency & duration of symptoms, what helps them, what makes them worse, past medical history and past history of similar problems, treatment or medications you tried, tests&results, bringing of pertinent records. Family history of similar disorders.

Provide a chronology all records and test results. Focus on your issues, do not give attention to prior doctor disappointment
