Unless your heart rate or blood pressure are super low from the propranolol I think you could do as directed by your prescribing physician. I have had patients do that very switch without incident.

Unusual to develop urticaria after having used it for"years". While Cardizem may be a reasonable replacement, I am hesitant to recommend switching without tapering first. If your blood pressure is under reasonable control, it may be reasonable to taper rapidly. Good luck.
