These EKG findings are quite abnormal but the severity depends on other physical findings as well as patient's age. There are several cardiopulmonary conditions which can cause this EKG change and the problem is likely chronic in nature.

Sinus tachycardia simply means a rapid heart rate which may or may not be serious. Right atrial enlargement considered as a possibility based on the ECG. Your best next step is to ask the doctor who ordered the ECG since he/she is familiar with the clinical problem. It is always best to have the ordering physician interpret these findings.
