Work up is needed and should be escalated to the brain level with a complete neurological exam followed by brain imaging. Would recommend starting/ continuing with your main doctor who can examine and coordinate further based on findings.

Appears you have gait imbalance which can be due to proprioceptive problems in spinal cord, cerebellar dysfunction, or other central nervous system problems. Your ears are unlikely to be the problem. Get your second opinion from an experienced neurologist. Blood tests and imaging are indicated.
