I am in pt for my neck. pain is not improving with muscle relaxers and exercises. i have straightening of the cervical lordosis, central canal stenosis, 3 herniated disc, effacement of ventral sac and uncovertable hypertrophy. why am i in still pain?

I'm usually reluctant to recommend neurosurgery on the neck, but it appears that you've been on good treatment and therapy for your complex cervical spine pathology. I recommend that you consult a neurosurgeon, if you haven't already done so, with interest and experience in complex cervical spine pathology. Consider consulting one at a department of neurosurgery at a university hospital near you.

Agree with Dr Colantino, and also suggest neurosurgical consultation. Regarding the rehab work, especially PT, you may not have participated a sufficient amount of time to "turn the corner". Discuss lack of progress with your therapist. It may be likely, however, that aggressive intervention may be the best approach.
