I'm not familiar with caper bark powder or what kind of inflammation pills you're taking, but if there's significant anal tenderness you could have an anal fissure or thrombosed hemorrhoid. For these I recommend sitting in hot water or applying hot compresses off and on. if anal pain persists get examined by a physician or a proctologist for diagnosis and appropriate treatment.
