My head feels ok when standing or sitting, but when i bend over forward, i get a sharp pain and pressure in the back of my head. what could be the cause? no fever or any other symptoms. i've tried a nerve block and imitrex which hasn't helped?

I would recommend that you talk further with your doctor to get more history about your headache. The positional characteristics of the headache and the character of the headache being sharp could mean that the pressures inside of your central nervous system need to be evaluated by imaging of the brain and perhaps a neurology consultation.
