I believe your wife would be best served by consulting an obstetrician who specializes in high risk pregnancies, perhaps at a university hospital near you. She must be treated as if the second baby will be Rh positive. If she received Rh immune globulin during the first pregnancy she will need it again. If she didn't she is already Rh sensitized and will need very specialized and careful care.
