Can having anti-kell cells from a transfusion 20 years ago cause problems with your immune system today? (eg: lowering it so you get sick easier, or cause problems like ra, lupus, etc?) thank you?

Anti-kell antibodies from a prior transfusion would not have any consequences or relation to your health issues. The only time it would be relevant would be, if you needed an other red blood cell transfusion, and even then, the blood bank will work around it. Wish you good health!
