I'm on nifedipine and have been for years and years, but last night my diastolic jumped to 145 and it's never been anywhere to before. should i be concerned?

You should verify the reading of a diastolic pressure of 145. Likely an error but I would have it checked by your doctor of in an emergency room. Suggest that you take your BP machine so that your doctor or the ER can make sure it is not a machine error.

I agree with Dr. Parameswaran. Was it a typo and you meant systolic pressure? If DBP was 145 what was the SBP (upper number)? People with diastolic pressure of 145 would be very symptomatic with headache, blurred vision and symptoms of stroke.
