Your question is not clear. What do you mean by "accurate after 17-18 years"? Do you mean after 17-18 years of suspected exposure?

If after 17-18 years of exposure, you have no symptoms and the test is negative, you do no have HIV. If you have any doubt, get a HIV nucleic acid test. Wish you good health!
